Do I have to fill out the intake form?
I highly recommend filling out your health history intake form in advance. This will save you valuable time during your first treatment which in turn saves you money. By filling it out in advance you can also spend time assessing your own history logging all past injuries, surgeries, hobbies and how you have used your body. This really helps to give a therapist a broad picture of how you have used and maintained your body and what may be contributing to your current issues
